Try some fine metal rod from a hobby shop for the shaft and shape some Milliput for the spear end. Sand the spear end to the right shape, prime and paint.

I think the nurse was made by Deauville Miniatures. See the image above. The soldier is Deauville's Imperial Yeomanry figure. I have a feeling the nurse was also sold as a New Hope Figure.
